In a candid interview with Vogue, the actress disclosed that she made the decision to have a hysterectomy, removing her uterus, fallopian tubes, and ovaries, in order to stop the production of estrogen in her body. This was a crucial step in preventing the cancer from returning, as luminal B breast cancer feeds and grows on estrogen.

Describing the decision as "the best" one for her, Olivia emphasized the importance of being present for her family. She expressed her desire not to miss out on any moments of motherhood, especially with her son Malcolm, whom she welcomed with John Mulaney in November 2021.

Olivia's hysterectomy marked her fifth surgery in her cancer journey, following a double mastectomy, lymph node dissection, reconstructive surgery, and a nipple delay. Before the procedure, she had been taking Lupron to suppress estrogen, but the side effects left her bedridden with debilitating exhaustion.
